The U.S. organ transplant system is facing unprecedented scrutiny, with investigative reporting exposing concerns about practices such as Normothermic Regional Perfusion (NRP) and Out of Sequence Organ Allocation. This talk will explore the fragility of the national transplant network in the face of intense media attention, the implications of these controversies on public trust and policy, and the urgent need for systemic resilience. Drawing from firsthand experience with OPTN modernization efforts, this discussion will provide an inside look at the ongoing transformation of the transplant system, the challenges of governance reform, and the path forward in ensuring equity, efficiency, and accountability in organ allocation.
